New Clinical Care Coordinator Appointed at Genesis Medical Associates

Jennifer S. Adiwinata has joined Genesis Medical Associates as Clinical Care Coordinator, who will be responsible for educating patients, their families and facility staff about available services and providers primarily in the post-acute care setting. 

Following the guidelines established by the Accountable Care Organization, the clinical care coordinator works to ensure proper placement of patients by gathering preadmission information, collaborating with internal (intake) and external (case managers, discharge planners) partners to ensure quality of service and implementation of an effective treatment plan.

In essence, the clinical care coordinator will ensure that Genesis patients receive an uninterrupted longitudinal care by coordinating all transitions along their healthcare continuum.

Adiwinata, who resides with her family in Baden, PA is a graduate of Notre Dame University’s St. Mary College. Adiwinata has also earned a Master’s of Public Management from Carnegie Mellon University.  She started her professional career as a staff nurse 24 years ago and spent 13 years as Assistant Director of Nursing at the Vincentian Home in McCandless Township and the last two years as Clinical Admissions Coordinator for Lutheran Senior Life.

Genesis Medical Associates is the largest independent group of primary care physicians serving the Greater North Hills. Genesis has 10 offices stretching north from the City of Pittsburgh to Cranberry Township in Butler County. Genesis has partnered with Preferred Primary Care Physicians, an independent primary care physicians group serving the South Hills to form Keystone Clinical Partners LLC, which is the first physician-owned organization in Western Pennsylvania to achieve recognition by Medicare as an Accountable Care Organization.
